This position is currently in Grande Prairie. You will be working in conjunction with our LPNs as well as work with HCAs that are supervised by the Personal Support Coordinators. Additionally, you will work with Allied Health, community paramedics, EMS, lodge & other community partners. You will be required to work collaboratively within a multi-disciplinary team as well as with the clients and their support networks. You will provide care to clients assessing unmet healthcare/nursing needs, create care plans and authorizations for nursing services including delegation of tasks to the HCAs and LPNs. You will assist the HCAs trouble-shoot issues with the medications assistance program (MAP) & work with the LPNs to identify risks to clients and possible interventions.
