Reporting to the Manager of Transitions, Juravinski Hospital and Cancer Centre, the Social Worker will conduct psychosocial assessments and develop treatment plans that include counseling, education and patient and family interventions. The Social Worker will work collaboratively with hospital team members, primary care, home care and other community providers to support patient centered care including seamless and timely transitions between hospital and community. Organizing and leading meetings with the multidisciplinary team to develop holistic care plans for patients with complex health and social needs will be a key responsibility of the Social Worker.
