Job description Position Summary As a key member of the Sunnybrook Academic Family Health Team, and an integral part of our interdisciplinary health care team, you will deliver outpatient nursing care in accordance with professional practice standards. You will deliver services to a variety of patient populations including high-risk groups suffering from chronic diseases and/or complex bio-psycho-social concerns. You will also provide health education to our patients with a focus on health promotion, disease prevention, and chronic disease management.
