DUTIES AND RESPONSIBILITIES: o Responsible for delivering fundamental care to patients in home or clinical setting. o Maintaining cleanliness in homes (light housekeeping). o Assisting with activities of daily living (ADLs) such as eating, dressing, toileting, bathing, and grooming. o Provide transportation, as required, for patients to attend clinics, programs and services provided in the community. o Assist Home Care Nurse with range of motion exercises, simple wound care, respiratory equipment, or urinary care. o Report patient condition and concerns to Home Care Nurse. o Assist patients to take their medications. o Communicate effectively with patients. o Document and record patient information to client record. o Other duties as required.
