Case Worker

The Street Worker's Advocacy Project is hiring a Case Worker for its Raising Hope program, a supportive residential program for pre and post natal substance using women and their children. The Case Worker is responsible for providing coaching, counselling, advocacy, referrals and crisis prevention/intervention. They will engage in goal setting with residents, helping them to identify challenges and supporting them to address these areas and achieve their goals; participate in case management meetings; provide transportation to appointments and activities; engage clients in financial planning; assist residents with building connections in the broader community to promote continued self-growth and wellness; and ensure the safety of residents and their children through the application of appropriate policies and procedures.
