Outreach Program Coordinator Full-Time Salaried | 37.5 hours/week Reports to: Community Connections Centre Manager Responsibilities: Promote housing stability and independence through assessments, support planning, advocacy, life-skills development and community referrals. Key Responsibilities Coordinate client intakes, assessments and housing transitions with the CCC Housing Coordinator. Develop and monitor individualized service plans and housing goals. Assist clients in securing and maintaining affordable housing, understanding tenant rights and addressing barriers to housing stability.
