This exciting opportunity as Registered Nurse, Designated Float covers planned and unplanned staff absences. Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.
