Durham West Family Health Team is seeking two compassionate, organized, and collaborative Physician Assistants to join our interprofessional primary care team. Based at Priority Health Clinic, the Physician Assistants will work under physician supervision to support timely access to care, continuity, and ongoing management of patients? health needs. These roles will support FHT clinic services, including the after hours clinic, new patient appointments and attachment, clinical inbox management, primary care, and chronic disease management. The successful candidates will help patients establish care and receive timely assessment, treatment, and follow up. *Key Responsibilities* *Primary Care and After Hours Clinic* * Conduct patient interviews, obtain health and social histories, and perform physical assessments. * Assess acute and ongoing health concerns and develop treatment plans in collaboration with the supervising physician. * Assess clinical findings and review diagnostic results within individual competence and applicable medical directives, escalating concerns to the supervising physician as appropriate. * Arrange investigations, treatments, and referrals through appropriate physician orders and authorized processes. * Provide patient education on treatment plans, medication use, preventive care, and follow up. * Support the after hours clinic by assessing patients with acute or episodic concerns and coordinating care with the supervising physician. * Recognize urgent findings, clinical deterioration, and concerns requiring immediate physician assessment or escalation. * Support continuity by documenting care and communicating relevant updates to the patient?s primary care provider. *New Patient Appointments and Attachment* * Support new patient intake appointments, including reviewing medical history, medications, allergies, preventive screening, and current health concerns. * Identify immediate care needs, gaps in care, and priorities for ongoing management. * Assist with medication reconciliation and obtaining relevant previous medical records. * Work with physicians and administrative staff to support patient attachment to a primary care provider. * Help patients understand available clinic services, how to access care, and next steps in their care plan. * Coordinate follow up appointments, investigations, and referrals to help patients establish ongoing care. *Clinical Inbox Management* * Review and prioritize assigned clinical inbox items, including investigation results, consultation reports, hospital records, and patient messages. * Identify abnormal findings and time sensitive concerns requiring physician review. * Follow up on results and outstanding investigations in collaboration with the supervising physician. * Communicate physician approved findings, recommendations, and next steps to patients. * Support prescription renewal requests through chart review, medication reconciliation, and identification of monitoring or reassessment needs. * Ensure clinical messages and follow up tasks are addressed, documented, and escalated appropriately. * Maintain clear responsibility for outstanding items and communicate any pending concerns during handovers. *Chronic Disease Management and Preventive Care* * Support assessment and ongoing management of patients living with conditions such as diabetes, hypertension, cardiovascular disease, asthma, and COPD. * Monitor symptoms, treatment response, medication adherence, and relevant clinical measures. * Collaborate with physicians and other healthcare professionals to develop and review individualized care plans. * Provide practical education to help patients manage their conditions and recognize changes requiring medical attention. * Identify preventive screening and immunization needs and coordinate appropriate follow up. * Recognize social, financial, functional, or other barriers affecting a patient?s ability to follow their care plan. * Connect patients w
