The Kidney Clinic and Renal Transplant PAA is an integral member of the renal program team. Summary of Duties: - Responsibilities include but are not limited to the following: - Collaborate closely with our pretransplant Nurse Clinicians, Data Manager, Renal Medical Director, Home Dialysis Medical Director and Home Dialysis/Kidney Care Clinic Patient Care Manager. - Work autonomously to complete tasks that require their input and or management. - Work collaboratively with other transplant centres to help process and navigate our patients requiring transplant work up and support. - Coordinate testing schedule, in collaboration with the patient and provides the patient with complete listing of tests. - Prepare and send patient samples to different health care labs and destinations - Organize and submit data in collaboration with the Data Ma...
