Job description

Review client referrals and conduct further assessment as required to identify client strengths, needs, concerns, and appropriate areas for support. Provide individual mental health counselling and therapeutic support to clients experiencing a range of mental health, wellness, and addictions-related concerns. Develop and maintain client care plans, complete appropriate documentation, and coordinate referrals to other services when required. Assess and respond to client safety concerns, including completing suicide risk assessments and safety planning within the scope of the position. Respond to clients experiencing emotional distress or crisis by providing immediate support, assessing safety and connecting clients with emergency or other appropriate services when a higher level of intervention is required. Work collaboratively as part of a multidisciplinary team to support the mental health and social well-being of community members. Participate in the planning and delivery of community mental health and wellness programming, workshops, presentations, groups, and outreach activities. Provide mental health education and presentations to schools, community programs, organizations, and other groups as requested. Collaborate with other service providers and community agencies to coordinate client care, referrals, and access to appropriate supports. Participate in program planning, community initiatives, and the development of services that respond to identified mental health and wellness needs. Maintain professional confidentiality and appropriate professional boundaries in accordance with legislation, professional standards, organizational policies, and mandatory reporting requirements. Other work-related duties as assigned by the Immediate Supervisor.
