The role supports the delivery of interdisciplinary outpatient services by coordinating referrals, triaging patients according to program criteria, managing waitlists, facilitating communication between patients and clinical teams, and supporting efficient clinic operations. The successful candidate will work closely with physicians, physiatrists, nurses, occupational therapists, physiotherapists, rehabilitation assistants, administrative teams, and external healthcare providers to ensure patients are appropriately assessed, prioritized, scheduled, and connected with the most appropriate rehabilitation services. The role also supports workflow improvement, service planning, data tracking, and reporting, identifying operational trends and opportunities to improve patient access, referral management, and clinic efficiency.
